It's been called the most difficult game on the internet. You're an "Olympic Runner," and that term is used loosely. You have to make the runner run by lifting/flexing his thighs and calves to make him run 100 meters, and it's just about impossible. You also flail around like your runner has no bones in your legs if you don't do it "right." Check out this video for an idea:

I've never made it more than 20 meters myself, and it was slow...and painful. Heres the best advice I've seen:
start by tapping O to start him leaning forward. When he's got a bit of forward momentum, hold P and Q together to make him take his first step. When his front foot is coming down, change to pressing O and W together to make him take his second step. Keep repeating, but try to time it so he stays leaning slightly forward.
