And yeah, I'm a surgeon in a big private practice so I'm in charge of a lot of people. I don't think I should be held solely responsible every time an employee makes a mistake. There's got to be personal responsibility all the way down the line
Internally, absolutely. You drill down and figure out as much as you can about what happened and why. Pick your preferred method, they'd probably all work just fine here.
If you are watching a surgery and you miss something, like a person not being in the OR when they should be, you wouldn't feel ultimately responsible? The missing person and people closer to them should be held accountable, but you are the one running the show, yeah?
